The offset option of mtools can be used to skip the header and first
segment of a DOS NBI, e.g. in ~/.mtoolsrc, put
drive t: file="/tftpboot/dos.nb" offset=4096
Then you can do mdir t:
Volume in drive T has no label
Volume Serial Number is 1DE9-302A
Directory for T:/
COMMAND COM 66663 12-19-1997 7:02
telbin exe 181776 08-30-1998 22:48 telbin.exe
ne1000 com 8751 08-29-1998 23:47 ne1000.com

mtype t:autoexec.bat, etc.
4096 is the value for the version mknbi-dos I used, being 512 for the
header + 3584 for the startup segment. It may be different for other
versions of mknbi-dos.
I think modify access won't work reliably, because the declared size of
the "disk" in the header will not be changed by mtools.
