Etherboot 4.2.4 released

I have released Etherboot 4.2.4. The changes are mostly bug fixes to the
RTL8139 driver: reducing the memory footprint and removing the polling
loop inside the driver routine; and some documentation cleanup. There
is a patch file from 4.2.3. The MD5 sums are:
